I understand that Haven Paws does not require pets coming into the facility to be vaccinated. I understand and acknowledge this risk and agree that Haven Paws is not liable for any expense related to illness suffered by your Pet during or after its visit.
Haven Paws is authorized to seek medical attention for my pet, and I will be responsible for any related expenses.
Pets with matted coats need extra attention during their grooming session. Mats can be difficult to remove and may require the pet to be shaved. Haven Paws reserves the right to take proper deshedding procedures and I will be responsible for any additional deshedding charges. Haven Paws Grooming does not wish to cause serious or undue stress to your pet and will not continually de-mat your pet for you. Mats can be very difficult to remove and may require the pet to be shaved. When necessary, removing a heavily matted coat includes risks of nicks, cuts or abrasions due to warts, moles or skin folds trapped in the mats. I understand that I am responsible for proper care of my pet after deshedding procedures.
Owners MUST inform Haven Paws if their pet(s) bites, has bitten, or is aggressive to people, animals, or specific grooming procedures. Muzzles may be used if necessary. Haven Paws reserves the right to refuse/stop services at any time.
Haven paws will not use pesticide dips or sprays on your pet. Ticks and pests found will be removed for an additional charge. If ticks are found, it is the pet owner's responsibility to seek proper medical advice.
I acknowledge that Haven Paws will take all reasonable precautions to ensure the safety and well-being of my pet. However, I understand that interactions with other animals may occur and I release Haven Paws from any liability arising from such interactions.
I understand that Haven Paws is not a professional Groomer, and the in the state of Minnesota a license is not required to perform grooming.
I am aware after the groom within 24 hours that if anything shall happen with health concerns with my pet and it is pre-existing condition Haven Paws is not responsible.
I hereby release Haven Paws, its owners, employees, and agents from any liability for injuries, illnesses, accidents, or damages that may occur during the grooming.
I consent to the unrestricted of all photographic or other images created by Pet Suites, and my pets name for use in social media & website placement, or other printed materials.
