I certify that my pet is in good health and understand that Haven Paws Grooming and Boarding does not require up-to-date vaccination records for other visiting pets.
Haven Paws Grooming and Boarding is authorized to seek medical attention for my pet, and I agree to be responsible for any related expenses.
I acknowledge that Haven Paws Grooming and Boarding will take all reasonable precautions to ensure the safety and well-being of my pet. However, I understand that interactions with other animals may occur, and I release Haven Paws Grooming and Boarding from any liability arising from such interactions.
I hereby release Haven Paws Grooming and Boarding, its owners, employees, and agents from any liability for injuries, illnesses, accidents, or damages that may occur during the boarding period, including transportation to and from the facility.
Haven Paws Grooming and Boarding is not responsible for providing food or treats during boarding. I understand that I, as the pet owner, will provide all food, snacks, collars, leashes, beds, and food bowls (if specific items are required).
I understand that Haven Paws Grooming and Boarding will not be responsible for any lost, damaged, or misplaced belongings of my pet.
I consent to the unrestricted use of all photographic or other images of my pet, as well as my pet's name, for social media, website placement, or other printed materials by Haven Paws Grooming and Boarding.
